A 2025 study in Nature Medicine followed 105,015 adults for up to thirty years to see who reached seventy in genuinely good health. Of eight eating patterns tested, the Alternative Healthy Eating Index had the strongest link, and the associations were generally stronger in women.
The data came from two long-running Harvard cohorts, including 70,091 women in the Nurses’ Health Study. Healthy aging meant reaching seventy free of eleven major chronic diseases, with cognitive, physical and mental health intact.
That bar is high. Only 9.3 percent of participants cleared it.
People in the top fifth for the index had 1.86 times the odds of healthy aging compared with the bottom fifth. When the bar moved to age seventy-five, the top fifth had 2.24 times the odds.
The foods behind it are not exotic. Fruits, vegetables, whole grains, nuts, legumes, unsaturated fats and low-fat dairy were linked to better odds.
Trans fats, sodium, sugary drinks and red or processed meats were linked to worse odds. People eating the highest share of ultra-processed food had about 32 percent lower odds than those eating the least.
Be clear about what this is. It is observational, the participants were health professionals, and physical, cognitive and mental health were self-reported.
Diet was measured repeatedly over decades, which is a real strength. Still, people who eat this way may differ in ways the researchers could not fully adjust for.
Here is the move. Pick two of the easiest parts of the score and make them daily: at least one serving of nuts or legumes, and zero sugary drinks or fruit juice.
A serving is one ounce of nuts, about a small handful, or half a cup of cooked beans or lentils. Swap sugary drinks and juice for water, sparkling water or unsweetened tea.
Next week, add one more: a whole grain in place of a refined one at one meal. Small swaps repeated daily are how an eating pattern gets built.
Nuts or legumes daily, and no sugary drinks or juice. That earns full marks on two of the eleven parts of the score that tracked with aging well.
